    Please ensure you have completed the Key Fob Undertaking document before completing the below form. Both the Undertaking and the below form must be completed before a new key fob can be issued. Complete Key Fob Undertaking

    Attachments Section

    Please use this section to provide the specified documents that allow us to verify your right to a key fob to the Dorset Square Garden. Your identification documents are temporarily stored in our CRM platform only, to be removed on completion of the processing of your request or automatically 21 days after submission if still not processed. Your key fob undertaking form, which represents a contract with the garden, will be stored in the same system indefinitely. Please ensure each attachment is no larger than 10mb.
